Major Charges/Projects

The Forms and Documentation Subcommittee is a subcommittee that reports to the Apheresis Applications Committee.

The Subcommittee is responsible for develop standardized forms for various procedures, with the ultimate goal of using these forms as a step in the direction of developing a national apheresis registry.

Currently, the Subcommittee is focussing on developing the following standardized forms:

• Orders sheet for apheresis
• Consult template/request for apheresis
• Run sheet
• Progress note/procedure record

Meeting Schedule

The Forms and Documentation Subcommittee meets monthly via teleconference.  The Subcommittee also meets in conjunction with the Apheresis Applications Committee monthly via teleconference and in person at the Annual Meeting.

ASFA members are invited to apply to serve on the Forms and Documentation Subcommittee by sending an email, stating their interest in joining the Forms and Documentation Subcommittee to asfa@apheresis.org.  Please include a brief CV as an attachment to your email.
